All girls participate in a behavioral/self-awareness groups, which are conducted by the group home counselor.
The modality of the group is geared toward positive peer culture and behavioral modification. Each girl works on a behavioral check list which is designed for her specific needs. Girls are asked to reevaluate these weekly. Group home counselor will evaluate the behavior checklist weekly. The group home staff "team" also reviews these for weekly evaluation. We will also offer social skills development and communication groups.
The program will be designed to provide case management services. At the time of the admission of any resident, they will be assigned the staff member responsible for providing case management services. Case management services will address:
1. Helping the resident and the parents or legal guardian to understand the effects on the resident of separation from the family and the effect of group living;
2. Assisting the resident and the family to maintain their relationships and prepare for the resident's future care;
3. Utilizing appropriate community resources to provide services and maintain contacts with such resources;
4. Helping the resident strengthen her capacity to function productively in interpersonal relationships;
5. Conferring with the child care staff to help them understand the resident's needs in order to promote adjustment to group living; and
6. Working with the resident and with the family or any placing agency that may be involved in planning for the resident's future and in preparing the resident for the return home or to another family for independent living or for other residential care.
B. The provision of case management services will be documented in each resident's record
Residents and families will receive individual counseling with a licensed mental health provider, utilizing out-patient services or with our licensed clinician.
At Gabrielle’s House it is our desire to create a positive environment where appropriate behavior skills are reinforce through positive reinforcement. Positive behavior brings positive consequences, negative behavior brings negative consequences.
We will work with resident's on how to gain four basic needs behavior skills are reinforce through positive reinforcement. Belonging, Mastery of skills, Independence and Generosity. We will assist youth in learning pro social skills that will help them to become more successful and assist in reaching their goals.
Girls are encouraged to be involved in the community. We prepare children to become positive members of the community. Our girl's learn important lessons about themselves and about the world around them through meaningful volunteer opportunities. Opportunities may include visiting nursing homes and helping at a food bank.
A variety of leisure and recreational activities are offered both on-site and in the community. Recreational group activities are planned to meet the girl’s interests and needs as well as to promote new interests. All activity planning includes input from staff and residents and is supervised by the staff. There is a structured schedule for weekly activities which includes a wide range of activities which relate to the residents’ ISP goals. For example, arts and crafts, cooking, sports, Girl Scouts, nature walks, and various seasonal activities.
After Care plans are designed for individual resident's needs and coordinated with the referring agents prior to discharge.
When discharge approaches, the case manager will manage the development of a Post Placement Plan. The Discharge Plan form will be used as a guide. When a resident is close to accomplishing their placement goals, as indicated by ISP in the program and goal accomplishment, a discharge site will be decided upon with input from the family and case worker.
Youth receive individualized services focused on enhancing, or supporting life skills to effectively transition into adulthood.
The girls who are accepted into the local public school system will receive their education through local schools and after school one-on-one tutors within the home . A trained special education staff member will advocate for our residents who have an Individual Education Programs (IEP). Our staff member will be present during school meetings to make sure the IEP’s are developed and implemented for meeting the academic, emotional, behavioral and social needs of each girl at Gabrielle’s House. Girls who are not accepted into our local zone school will attend an alternative school. Our zone schools are Providence Middle School and James River High School.
Finding – and keeping – a job gives children more confidence and self-reliance, teaches them to be financially responsible, and gives them skills that they will carry throughout their careers. Girl’s will have the opportunity to explore different careers through vocational paths. Gabrielle’s House encourages employment, supports the development of job skills, and supervises money management. Resident’s can work 10-15 hours per week, while going to school. During the summer they are able to maintain full time employment. The girl’s will be encouraged to save most of their earnings, toward moving out on their own.
Services include routine physical check-ups, neurology, dental/vision/hearing screenings and follow-ups, recommended lab testing and other physician-prescribed evaluations. These services are provided directly through physicians office visits. Emergency medical services are accessed through local hospitals/E.M.T. services.
Health and physical fitness are a major focus in providing a well-rounded program. Girl's participate in a recreation therapy program promoting exercise, sportsmanship and a sense of self- improvement. Students are instructed in healthy eating habits and the benefits of the four major food groups.
