Admission's Information

THERAPEUTIC GROUP HOME REFERRAL PROCESS

 The process of Referrals for any of home  begins with a submission from a behavioral health network representative, a probation officer, or a Child Protective Services Case Manager. All referrals should be sent to hello@optimumyouths.com
Referrals may be made by telephone or in writing via email or mail.  The following information will be required at the time of referral:· General information regarding reasons for referral, acting out behaviors and presenting problems· Social history, abuse history, and any criminal history· 

In addition, History of aggressive behaviors· Previous discharge plan· Placement critical goals
For other inquiries of the GABRIELLE'S HOUSE Programs or questions regarding eligibility, funding or admission criteria, please contact us.
Once initial screening is done by phone, further background information will be requested such as:·

 Psychological evaluation, · Family history · Reports from previous interventions,  a  pre-placement interview will be scheduled. When material is received, then a review of the paperwork is completed,. 

If the child seems to fit the placement and the Program Director or Administrator reviews the full file, we will contact the Agency and arrange for a pre-placement visit with the child, worker and or the possibly the natural family . In addition, to complete the process, we require:

  • Completed Application 
  • Current and complete psychiatric history and hospital records
  • Current physical exam, TB test and tetanus shot (within 5 years)
  • Tour of the home and interview with the admissions committee

We ensure that all candidates are evaluated by consistent standards and in an unbiased review process; we screen and interview each applicant following the review of all attainable previous records and referral information. The admissions process is geared toward admitting youth who will benefit from our environment and willingly participate in its therapeutic environment. Acceptance Criteria

  • Adolescent boys and girls ages 11-17
  • Adolescent boys and girls who have Incarcerated Parent(s)
  • Adolescent boys and girls who have Behavior Disorder
  • Adolescent boys and girls who have Learning Disabilities
  • Adolescent boys and girls with an IQ of 70 and above.
  •  Adolescent boys and girls diagnosed with ADHD   
  • Adolescent boys and girls who have experience trauma
  •  Adolescent boys and girls who are Bi-polar  
  • Adolescent boys and girls who are Emotionally Disturbed
  • Adolescent boys and girls who demonstrate a capacity to respond favorably to rehabilitative counseling and training in areas such as problem solving, life skills development, and medication compliance training such that reintegration into the family unit or a foster home is a realistic goal.
  • Adolescent boys and girls who are suicidal
  • Adolescent boys and girls who have Delinquent/Court Involvement
  • Adolescent boys and girls  who can function in and out of a  local public school setting, both behaviorally and academically (children with special education needs.

  • Adolescent boys and girls who Physical Handicaps associated with ambulation, sight, hearing, speaking, etc; that would prevent the applicant from being able to function in program
  • Adolescent girls who are pregnant
  •  Adolescent requires a level of structure and supervision beyond the scope of the program.

  • Completion of a High School Diploma, GED, or Vocational Program ( ages 16-17)
  • Secure a relationship with a mentor 
  • Develop coping skills to help with their trauma
  • Demonstrate independent living skills
  • Complete community service and volunteer projects.
  • Develop workplace skills by securing employment (ages 15-17)
